DHI hair transplant

"DHI" appears on many clinic websites in India, often with a higher price than standard FUE. The term is used loosely, so it helps to know what it usually means before you compare quotes. This guide explains what DHI generally refers to, how it relates to FUE, what the evidence does and does not show, and what DHI usually costs in India. It is general information, not medical advice. A qualified doctor who examines you should decide which technique suits you and how many grafts you need.

What DHI means

DHI usually stands for direct hair implantation. In most clinics it describes a way of placing grafts, not a different way of taking them. The grafts are removed from the donor area one by one, exactly as in FUE. They are then loaded into a pen-like instrument called an implanter, which puts each graft into the scalp.

The most widely known implanter is often called the Choi implanter. A 2024 review in Facial Plastic Surgery says Dr Choi introduced it in 1992, and calls it the Korean implanter. It has a hollow needle that holds one graft. The needle is pushed into the scalp at the chosen angle and depth, and a plunger releases the graft. With this tool, making the opening and placing the graft can happen in one movement. In standard FUE, the openings are usually made first, and the grafts are then placed with fine forceps.

So in practice, a "DHI" procedure is usually FUE extraction followed by implanter placement.

Why the term is confusing

There are three reasons to ask what a clinic means by DHI:

• It is used loosely. Some clinics use "DHI" for any procedure that uses implanter pens. Others use it for a particular protocol, and some use it for FUE with only part of the placing done by implanter.
• It is also used as a brand name. At least one hair-restoration business uses DHI as part of its name. So "DHI" in an advert may refer to a company as well as a technique. This guide describes the technique only and makes no claim about any company.
• Similar names are used as brands. Terms such as "direct hair transplant" appear as clinic brand names too.

When a clinic offers DHI, ask it to explain in plain words how grafts will be removed, how the openings will be made, and who will place the grafts.

How a DHI procedure works

The steps are much the same as FUE.

1. Consultation and planning. A doctor examines you, agrees the area and the number of grafts, and checks your health. Blood tests are usually done beforehand.
2. Local anaesthetic. Your scalp is numbed and you stay awake.
3. Extraction. Grafts are removed one at a time from the donor area with a small punch, as in FUE. The donor area is usually shaved.
4. Loading. Team members load each graft into an implanter, often working with several implanters in turn.
5. Implanting. The doctor or trained team inserts each graft with the implanter at the planned angle, direction and depth.
6. Aftercare. You go home the same day with instructions for washing, sleeping and medicines.

DHI compared with standard FUE

The 2024 review in Facial Plastic Surgery describes the implanter's advantage as making the opening and placing the graft at the same time, and notes that forceps also allow safe placement in well-trained hands. It concludes that the choice depends on the surgeon and the advantages and disadvantages of each tool. We did not find good-quality studies showing that DHI gives better final results than well-performed FUE. The experience of the team, the planning and the aftercare are likely to matter more than the name of the technique.

What DHI costs in India

Clinics in India usually price DHI higher per graft than standard FUE. "DHI" is often marketed as a premium option, and implanter placement can need extra instruments and trained staff.

Our national estimate for DHI is ₹50 to ₹100 per graft, with a wider market range of about ₹40 to ₹150 per graft. For comparison, standard FUE is estimated at ₹30 to ₹60 per graft.

These are estimates, not quotes, and they exclude GST. Because clinics use "DHI" in different ways, published DHI prices vary more than FUE prices. Some quotes called DHI may be FUE with implanter placement for part of the session. For very large sessions we apply a lower rate per graft, based mostly on FUE prices, so treat the larger bands as a rough guide.

When you compare a DHI quote with an FUE quote, check:

• whether the graft count is the same
• who does the extraction and who does the placement
• what is included, such as tests, medicines, follow-ups and PRP (PRP is often ₹4,000 to ₹9,000 per session)
• whether GST is added

Before you choose

A hair transplant is surgery, whatever it is called. The ISHRS says any procedure that cuts the scalp should be done by a licensed, trained doctor, not by technicians working alone. Ask for the doctor's name and registration number, which you can check on the Indian Medical Register, and ask what their role will be on the day.
