Second hair transplant: when it makes sense

Many people have more than one hair transplant. The International Society of Hair Restoration Surgery (ISHRS) explains why: pattern hair loss keeps going, so a follow-up session, or a series of them, may be needed to keep the coverage you want. Here is when a second session makes sense, and what it may cost.

Why people have a second session

The ISHRS lists these reasons:

• hair loss has continued around the first transplant
• you want more density in the area already treated
• the hairline needs moving to suit your age

How long to wait

Wait until the first transplant has grown in. The American Academy of Dermatology (AAD) says most people see results 6 to 9 months after surgery, and some wait 12 months. Indian dermatology guidelines say proper growth can be expected after about 9 months. So a second session usually comes at least 9 to 12 months after the first. Judging density any earlier means judging hair that has not finished growing.

How much donor hair is left

Donor hair does not grow back once it is taken, so each session uses up part of a fixed supply. A study of 580 Indian men estimated what an average donor area can give:

These are averages, and yours may differ. The ISHRS warns that hair loss can outpace the donor supply, so that at some point there is not enough donor hair left. If donor hair runs short, some doctors add beard or body hair. See body hair transplant.

What a second session costs

Ask whether the clinic that did your first session prices a second session differently, and get the graft count and price in writing, as you did the first time.

Questions to ask before a second session

• How many grafts did my first session use, and how many can my donor area still give?
• Which areas will this session treat, and what might I need later?
• Will you take grafts by FUE from the same area, use FUT, or add beard or body hair?
• What will it cost, in writing?
