What age is right for a hair transplant?
Last updated 27 September 2026
There is no single best age written into Indian guidelines. What doctors look at is whether your hair loss has settled into a pattern, how much donor hair you have, your health and your expectations. Age matters because it changes the first of these.
What the Indian guideline asks
The Indian Association of Dermatologists, Venereologists and Leprologists (IADVL) says a transplant can be done in anyone with pattern hair loss, a good donor area, good general health and reasonable expectations. It asks for caution in very young patients whose early hair loss is still changing.
In your 20s
The American Academy of Dermatology (AAD) says a man in his 20s may be asked to wait and start a hair loss medicine first. There are three reasons:
- The loss keeps going. Pattern hair loss is progressive and hard to predict, so hair behind a new hairline can thin later and leave the transplanted hair on its own.
- Donor hair is limited. Grafts used now are not there later, when you may need them more.
- A low hairline ages badly. The International Society of Hair Restoration Surgery (ISHRS) says younger patients often push for a low hairline, and such hairlines are a common reason for repair work.
If you go ahead, ask for a hairline that will still suit you later.
In your 30s and 40s
The same tests apply: a settled pattern, enough donor hair and a plan for later. Most people still need medicine afterwards: the IADVL guideline notes that pattern hair loss is progressive and that most patients need medical treatment alongside surgery.
After 50
The guideline sets no upper age. What matters is your health, so tell the doctor about every condition you have and every medicine you take. The ISHRS notes that FUT can suit older patients, because scars tend to heal better with age. Ask for a hairline that suits your age. The ISHRS names moving a hairline to make it "age appropriate" as one reason people have further surgery.
What your age means for cost
The younger you are, the more years your hair loss has to progress, so a later session becomes more likely. Plan for the whole path, not only the first bill:
| Stage of the plan | Estimate for India, FUE, before GST |
|---|---|
| First session at grade 3, about 1,000 to 1,600 grafts | ₹30,000 to ₹95,000 |
| First session at grade 5, about 2,000 to 2,800 grafts | ₹60,000 to ₹1.7 lakh |
| A later session, about 1,000 to 1,500 grafts | ₹30,000 to ₹90,000 |
Medicines and PRP are extra. See cost by grade and second hair transplant, or try the cost estimator, which asks your age band.
Questions
What age is right for a hair transplant?
There is no fixed age. Doctors look at whether your hair loss has settled, your donor hair, your health and your expectations. They are more cautious in the early 20s, when the pattern is still changing.
Can I get a hair transplant at 21?
It is possible. Doctors are more careful at this age, because early hair loss is still changing. If you go ahead, ask for a hairline that will still suit you later.
Is 50 too old for a hair transplant?
No. The Indian guideline sets no upper age. Your general health, your donor hair and your expectations matter more than your age.
Should I take medicine before a hair transplant?
Ask your doctor. The IADVL guideline says most patients need medical treatment alongside surgery, because pattern hair loss keeps going.
